License Description
DESCRIPTION OF OCCUPATION: A Physician is a doctor of medicine who diagnoses and treats mental or physical disorders. They also advise people on good health practices. May specialize in a particular field of medicine. REQUIREMENTS FOR LICENSING: Personal requirements: Be a U.S. citizen, U.S. national, or alien authorized to work in the United States. Professional requirements: 1) File a National Practitioner Data Bank report or an affidavit. 2) Submit a Federation disciplinary history report. 3) Submit an American Medical Association physician profile. 4) Provide evidence of hospital affiliations for the last 3 years. Experience requirements: One year of residency training in an Accreditation Council for Graduate Medical Education (ACGME)-accredited program in the U.S. or one year of residency training in an RCPSC or CFPC-accredited program in Canada. Education requirements: M.D. degree from a Liaison Committee on Medical Education (LCME)-accredited medical school in the U.S. or Canada. EXAMINATIONS: National Board of Medical Examiners (NBME), Federation Licensing Exam (FLEX), United States Medical Licensing Exam (USMLE), MCCQE (Qualifying Exam of LMCC). FEES: Between February 1, 2022 and January 31, 2023, pay $392 (application $50, license $97, compliance resolution fund $148, ½ renewal for 2nd year of 2-year license $97). Between February 1, 2023 and January 31, 2024, pay $221 (application $50, license $97, compliance resolution fund $74). OTHER: There were 5,313 licenses currently active as of February 24, 2022. ~ revised 11/18
