Pharmacist
License Description
To obtain either a license or certificate, candidates must: Be at least 18 years of age. Have either graduated with a professional degree from a school of pharmacy accredited by the American council of Pharmaceutical Education or have graduated from a school of pharmacy located outside the United States and Canada, provided additional requirements have been satisfied. Have satisfactorily completed either a pharmacist intern or pharmacist extern program approved by the board. Not have been convicted of a crime nor have been subject to disciplinary sanctions that would interfere with the ability to carry out professional duties. Successfully pass the board-administered examination. Pay all application and examination fees. Licenses for pharmacists are renewed every two years, subject to continuing education requirements. Pharmacist intern and extern certificates are valid for six years. More detailed information, including application materials and test dates, may be obtained directly from the Indiana Board of Pharmacy.
