HFA Provisional
License Description Sec. 14. (a) An individual may be issued a provisional HFA license for a specific licensed health facility if the individual has: (1) at least two (2) years of administrative experience in a licensed HF; and (2) complied with the conditions of IC 25-19-1-3(a)(1). (b) An individual may be issued a provisional RCA license for a specific licensed residential facility if the individual has: (1) at least two (2) years of administrative experience in a licensed HF; and (2) complied with the conditions of IC 25-19-1-3(a)(1). (c) Under subsections (a) and (b), the director of the board may issue a provisional license to an individual who appears to be qualified; however, this license will be subject to the approval of the board at its next following meeting, at which time the board may withdraw the provisional license if it determines that the licensee fails to meet all applicable qualifications for the license. (d) Experience gained under provisional licensure shall not satisfy the educational or AIT requirements of section 4(a) or 4(d) of this rule.
