License Description End Stage Renal Dialysis Clinic. Fees: $0 fee for initial and renewal certification process. Examination Frequency: One time Exam Locations: Statewide and National Experience Requirement: Have a high school diploma or equivalency or have greater than four years of experience in dialysis as of October 14, 2008. Have successfully completed a CDPH approved hemodialysis training program. Have successfully passed a standardized test approved by the CDPH or the examination offered by a national commercially available certification program. Average Time to Process Application: 4 weeks. Renewal Period: Every 4 years. Special License Requirements: N/A Authority: California Business and Professions Code, Section 1247-1247.9.
