License Description Certification of an operator or manager of a food establishment Required Documents: Proof of passing an approved exam, the fee, and a completed application. State to state reciprocity may be possible. Prerequisites: Taking and passing a state approved examination. In most cases, exams are included as part of the certification course. For information about available courses, call (608) 266-2835 or (608) 261-8361. or email to plicESS@dhfs.state.wi.us Current approved courses are: ServSafe (SS) Examination by the Educational Foundation of the National Restaurant Association The National Registry of Food Safety Professional, Food Safety Manager Certification Examination National Assessment Institute's (NAI) Certified Professional Food Manager Examination from Experior Assessment, Clearwater, Florida Dietary Manager's Association (DMA) Food Sanitation and Safety Examination from the Certifying Board of the Dietary Manager's Association.
